Informed Consent; and Office Policy

I. Appointments:

Appointments are usually scheduled for 45 to 50 minutes. Patients are generally seen weekly or more/less frequently as needed and you and Dr. Bruno agree. You may discontinue treatment at any time, but please discuss any such decisions with your psychologist.

II. Confidentiality:

The law protects the relationship between a client and a psychotherapist, and information cannot be disclosed without written permission.

Exceptions include:

  • Suspected child abuse or dependent adult or elder abuse, for which I am required by law to report this to the appropriate authorities immediately.
  • If a client is threatening serious bodily harm to another person/s, I must notify the police and inform the intended victim.
  • If a client intends to harm himself or herself, I will make every effort to enlist their cooperation in ensuring their safety. If they do not cooperate, I will take further measures without their permission that are provided to me by law in order to ensure their safety.
  • Dr. Bruno uses best practices for keeping client confidentiality. Yet, outside factors that may impact patient confidentiality might include:1.) if your psychologist is ordered by a court to release information as part of a legal involvement in certain litigation, 2.) if you use insurance, your insurance company can demand access to some of your records, (e.g. in filing claims, insurance audits, case reviews or appeals), 3.) in unforeseen natural disasters protected records may become exposed or 4.) when otherwise required by law. You may be asked to sign a Release of Information so that your psychologist may speak with specific professionals or to family members.

    In the event of a life-threatening emergency, call 911. As Dr. Bruno may not be readily available at 650-738-0807, you may call your primary care physician or the local emergency room, or a 24-hour crisis hotline, such as 650-579-0350 or 1-800-273-TALK.

    III. Record Keeping:

  • A clinical chart is maintained describing your condition and your treatment and progress in treatment, dates of and fees for sessions, and notes describing each therapy session. Your records will not be released without your written consent, unless in those situations as outlined in the Confidentiality section above. Medical records are locked and held strictly confidential.

    IV. Cancellations and Missed Appointments:

  • You will be billed for a sessions that you cancel with less than 72 hours notice. You may leave recorded voice messages at 650-738-0807, 24-hours per day. You will be billed $50.00 for missed appointments or late cancelations. Insurance companies do not reimburse for missed or late canceled appointments.

    V. Payments:

  • Payment is due at the time of the session unless other arrangements have been made. Dr. Bruno’s office will file or provide insurance claims, when requested and agreed upon, but you are responsible for deductibles, co-payments or refused coverage. It is your responsibility to familiarize yourself with your insurance benefits and to keep Dr. Bruno’s office informed of any changes in your insurance status or coverage.

    VI. Private Pay Fees:

  • Each 45-50 minute session (psychotherapy or psychological testing) thereafter is $150. Insurance companies which Dr. Bruno has an “in-network provider” contract pay at lower rates.

    VII. Legal & Expert Witness Fees:

  • Fees for Dr. Bruno to act as a forensic expert witness or when called for a court subpoena to provide testimony regarding a client’s legal case, is charged at $225 per hour, with services including travel time, testimonial or witness preparation time, and time waiting to be heard at the court. If the opposing party does not assume responsibility, then the client themselves are responsible for these fees.
